Resident Services Coordinator at Welsh Mountain Home Personal Care

Help Residents and Families Feel Supported

• Guiding prospective residents and families through the inquiry and admission process
• Developing strong relationships with hospitals, healthcare providers, social service organizations and other referral sources
• Coordinating admissions with Personal Care, Dining, Facilities and Finance
• Serving as a dependable resource and advocate for residents and their families
• Establishing relationships with residents’ family members and powers of attorney
• Coordinating resident services such as insurance assistance, representative payee services, voter services and Resident Council meetings
• Supporting activities, resident outings and opportunities for meaningful engagement
• Working collaboratively with team members to resolve resident concerns with patience and compassion
• Staying organized while managing multiple priorities, frequent interruptions and changing needs
• Helping Welsh Mountain Home maintain a healthy balance of private-pay and subsidized residents so its mission can continue

Qualifications

• High school diploma required; a bachelor’s degree in social work, psychology, health services or a related field is preferred
• Recent experience in a long-term care setting, particularly in admissions or as a clinical liaison
• Ability to obtain Personal Care Aide and Medication Technician certifications within 90 days of hire
• Ability to obtain a Personal Care Home Administrator license within the first two years of employment
• Strong verbal, written and interpersonal communication skills
• Ability to work independently, remain organized and manage multiple responsibilities
• Comfort using computers and other technology required for the position
• Flexibility to occasionally work evenings or weekends for resident and family events, conferences or other community needs
• Willingness to participate in an on-call rotation and provide support where needed
• A calm, respectful and compassionate approach when working with residents, families and team members
